Explore Randomized Floors

The game is structured as a roguelike dungeon crawler made from interconnected rooms. Isaac attacks by firing tears in four directions while moving independently around enemies and hazards. Most rooms lock until every enemy has been defeated, after which the exits open and exploration can continue. Runs pass through areas such as the Basement, Caves, Depths, and Womb, while additional routes and endings become available through continued progress.

Rooms can contain:

  • regular enemies and stronger champions;
  • treasure rooms with passive or active items;
  • shops that use collected coins;
  • locked rooms requiring keys;
  • secret rooms hidden behind certain walls;
  • bosses at the end of major floors.

Bombs can destroy rocks, damage enemies, or expose hidden entrances. Keys open treasure rooms and other locked areas, while coins are used for shop purchases. Hearts restore health, but some items replace or supplement normal red hearts with other forms of protection.

Build a Different Isaac Every Run

Items are the main source of progression inside a run. Some increase damage, range, movement speed, fire rate, or health, while others completely change how Isaac’s tears behave. A projectile can become larger, split into additional shots, pass through enemies, explode, curve, or gain status effects. Familiars may follow Isaac and attack automatically, while active items provide abilities that recharge after clearing rooms.

Because item combinations stack, two runs can play very differently even when they begin with the same character. One build may rely on rapid tears and speed, while another develops around high damage, familiars, bombs, or unusual projectile effects.

Defeat Bosses and Unlock More Content

Each major floor ends with a boss fight. Early opponents establish basic movement and attack patterns, while later bosses combine projectiles, summoned enemies, charging attacks, and restricted arena space. Defeating them usually provides another item and opens the route downward.

There is no conventional level-select campaign where completed stages stay identical. Progress comes from finishing runs, discovering endings, unlocking characters and items, and learning how different enemies and item combinations work. The main challenge is adapting to what the current dungeon provides, deciding when resources should be spent, and turning a random collection of upgrades into a build strong enough to survive the deeper floors.
