Taylor Lee, a 19-year-old university student, wakes up inside Krueger Health Solutions and learns that they are about to undergo “Dream Therapy” with Dr. Albert Krueger. Taylor does not remember agreeing to the appointment, but the doctor continues with the session anyway. The story gradually reveals that the therapy is not what Krueger initially claims and that his interest in Taylor is connected to the real purpose of his experiments.

A Therapy Session Made of Tests

Gameplay combines visual-novel conversations with several interactive psychological tests. Players read dialogue, choose responses, and complete tasks given by Krueger. The tests include basic mathematics, a cups-and-balls exercise, the Gateway to Your Heart sequence, and a Rorschach-style inkblot test. The game watches how closely the player follows Krueger’s instructions, and these actions influence which ending becomes available.

Following Orders or Deliberately Disobeying

There are no conventional levels, combat sections, character statistics, or equipment upgrades. Instead, each test acts as a separate part of the therapy session. Important gameplay actions include:

  • Answering mathematical questions
  • Tracking the correct cup during a test
  • Deciding whether to keep your eyes closed
  • Examining and flipping Rorschach cards
  • Choosing dialogue responses
  • Following or ignoring Krueger’s instructions
  • Replaying sections to change previous decisions

After the first playthrough, additional replay options make it easier to revisit individual sections and work toward another outcome.

The choices are not simply cosmetic. Consistently obeying Krueger produces a different result from intentionally failing his tests and refusing to cooperate. Some interactions also change on later playthroughs, giving the player more information about Krueger, Taylor, and what is actually happening during the session.

Reaching All Three Endings

Therapy with Dr. Albert Krueger contains three endings: two bad endings and one good ending. The first is commonly reached during an initial playthrough, while the other outcomes require more specific behavior. Perfectly following instructions leads toward one route, whereas deliberately answering incorrectly, opening your eyes when told not to, and interacting differently with the final inkblot are required for the good ending. Seeing all three endings provides a fuller explanation of the story and Dr. Krueger’s intentions.
