Horrible Therapist is an adult party card game about constructing three-panel comics set during therapy sessions. Each comic combines a Question, an Answer, and a proposed Treatment. Establishing the Therapy Session

Before play, the three decks are separated, shuffled, and placed face down. Every participant draws five Treatment Cards and keeps them hidden. One person becomes the Therapist for the opening round. That player temporarily sets their Treatment hand aside, reveals one Question Card, and places it as the first comic panel. The Therapist then draws three Answer Cards privately and selects one to form the second panel.

Completing the Final Panel

After the Question and Answer are visible, all remaining players act as patients proposing a treatment. Each chooses one card from their hand that they believe completes the comic effectively. These cards are submitted face down so the Therapist cannot identify their owners. The Therapist shuffles the submissions, reveals them individually, and reads the entire three-part comic with each possible ending before selecting a winner.

A complete round includes the following actions:

  • Reveal one Question Card as the opening panel.
  • Draw three Answer Cards and choose one.
  • Remove the two unused Answer Cards from the game.
  • Submit one Treatment Card from every non-Therapist player.
  • Shuffle all submitted cards to preserve anonymity.
  • Read each possible comic from beginning to end.
  • Award one point to the player whose Treatment is selected.

Points Replace Traditional Levels

Horrible Therapist does not contain numbered levels, locations, character upgrades, or a fixed narrative route. Its progression consists of short rounds in which the role of Therapist passes clockwise. The winning Treatment Card remains in front of its owner and represents one point. All participants then refill their hands until they again hold five Treatment Cards, while used Question, Answer, and unsuccessful Treatment Cards are removed from the current round.

The first player to collect three winning Treatment Cards wins the game. Success depends on understanding what the current Therapist is likely to choose rather than finding an objectively correct combination. A Treatment may be selected because it connects logically with the first two panels, changes the meaning of the conversation, or creates an unexpected conclusion. Players should compare all five cards in their hands with both visible panels instead of focusing only on the Question. Since the judge changes after every round, the preferred style of answer may also change throughout the game. Some cards include blank spaces that can be filled in before use, allowing players to introduce names, objects, or details suited to the group.
